Kindergarten Orientation June 13, 2007

Dear Kindergarten Parents:

            Welcome to Lincoln School.  As your child’s school nurse, my priority is the health and well being of the students. The attached list of health record requirements must be submitted before your child’s registration is considered complete. Please note the following standards set by Hasbrouck Heights Board of Education policy and New Jersey state law.

  1. For Physical examinations: an approved district form or equivalent is required and it must be dated and signed by the medical provider. The provider’s office must stamp the form giving its’ name, address and phone number.
  2. Immunization records must include the signature of the physician or advance practice nurse. We have available Medication and Religious Exempt Forms.
  3. Dental exam information may be added to the physician exam form, the dentist may use his/her own form, or you may use the form provided by the district. All must include the date, dentist’s name and signature. (Dated Six Months from the first day of School)
  4. Evidence of Mantoux tuberculin skin testing within the past 6 months for students entering the school system for the first time, if born in a high TB incidence country (See School Nurse for details.) Once again Religious Exemptions may apply.

Please make sure your emergency cards are completed, including cell phone numbers, to ensure effective communication for the well being of your child.

If your child has any medication requirements or conditions such as allergies please pick up the medication package (medication policy and forms) and call me with your questions. Please DO NOT send your child to school with prescription or over the counter medication or supplements. You must bring the medication in to the nurse. This includes cough drops which the student may take with a parent permission note.
